A: The policy expires on August 15, 2008 (or when you become ineligible for
coverage under the terms explained in EFFECTIVE and TERMINATION DATES.
Q: If I missed the original registration or waived coverage initially and now
want the coverage, how do I go about getting it?
A: Under certain circumstances, you may join the plan after the initial

Q: How do I know if I am eligible to take the Student Accident & Sickness
Insurance?
A: All registered full-time students of Ave Maria University are eligible and
will automatically be included, unless proof of adequate alternate coverage is
provided during registration. Students who enroll may also enroll dependents
during the open enrollment period. See ELIGIBILITY for details.
Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, if you are enrolled too. Please see ELIGIBILITY in the brochure for

Q: What does this insurance cover?
A: This is Excess Insurance. That means this insurance will pay secondary to
other insurance available. If you do not have any other insurance, this plan
will pay for medical care related to a covered Injury or Sickness. This plan is
for treatment of injuries and for sickness, it does not cover wellness visits
and it does not cover elective treatments. See the plan brochure for the
conditions, exclusions, the schedule of benefits and limits. If you have

Q: Does this policy pay all medical bills in full?
A: In many cases it will not. This is a limited benefit plan for injury or
sickness. This plan does not always pay medical bills in full. The plan has
limits and exclusions, so please read your plan brochure carefully. The plan
will pay benefits at Usual & Customary, up to the limits within this plan.
You are encouraged to seek providers who participate in the optional PPO
network, First Health. When you use those providers, you will often receive
reduced charges for the care and it will assure you the charges are within the
Usual & Customary charges for the care provided. This can reduce the amount you
will owe the provider for portions not covered by this plan.
Q: Can I go to any doctor?
A: Yes, you may go to any doctor. However, there is an optional PPO, First
Health. You may look up a provider anywhere in the country by going to the link
in this web site to locate a doctor or you can call First Health to search for a
local provider at 1-800-226-5116.
Q: Where do I get a claim form?
A: You may obtain a claim for on this web site by clicking the link to get a
claim form and printing it. You may also contact Bollinger to have a claim form
sent to you. A supply of forms will also be sent to Ave Maria University’s
Student Life Office.
Q: How do I file a claim?
A: The instructions for filing a claim will be on the back of the claim form
and they are also on the plan brochure under CLAIM PROCEDURE. Please be sure to
file a claim form to notify the carrier of an Injury or Sickness within 30 days
of an Injury or first treatment for a Sickness. Bills must be provided at that
time or at least within 90 days of service (or as soon as reasonably possible).
Q: Does this policy cover me for my annual physicals?
A: No. This policy covers doctor visits for Injury or Sickness. Further,
visits to a doctor on an Outpatient basis is not covered until the second visit
and limited to $50 per visit (1 visit per day).
Q: Is there a deductible on this policy?
A: No.
Q: With this insurance must I go to the University Health Center first?
A: No.
Q: What if I am out of state when I get sick or injured?
A: You are covered under this plan wherever you are. The optional PPO network
is national, so you may look up a provider on the link to First Health to search
for a doctor wherever you are in the U.S. by city or zip code.
Q: How do I get an I.D. card?
A: I.D. cards will be provided as soon as the University provides the list of
names and the cards can be created and mailed back to the University to
distribute. You may also print a temporary ID card from this web site, which
includes the info needed by a care provider to know the information about this
plan.
Q: What is the policy’s maximum benefit?
A: The policy maximum benefit is $50,000 per Injury or Sickness, with internal
limits stated in PART B, followed by MAJOR MEDICAL limits in PART C. Please

Q: What do you pay for doctor’s visits?
A: Doctor’s Non-surgical Visits on an Outpatient basis is not covered until
the second visit and limited to $50 per visit (1 visit per day). Doctor’s

80% of Usual & Customary under PART C. For Metal and Nervous Disorders,
