A: All students enrolled at Hollins University are included in this insurance
plan and the premium for coverage is added to the tuition billing unless proof
of comparable coverage is furnished.

A: This policy provides coverage for Injury and Sickness while you are insured.
Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will
not. There are exclusions and limitations in the policy. Please refer to the

Q: Can I go to any doctor?
A: Yes. However, if you see a physician from First Health, our nationwide
network, your fees and out of pocket costs will be less. You can obtain a

Q: Where do I get a claim form?
A: You can pick up a claim form at the Hollins University Student Health Service
Office or you can print one from this link.
Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O.
Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or
illness. Subsequent itemized bills may be sent in without completing another
claim form. Just indicate that you are a Hollins University student and the date
of your Injury or illness.
Q: Does this policy cover me for my annual physicals?
A: No, the program is designed to cover injuries and sicknesses while you are
insured.
Q: Is there a deductible on this policy?
A: Yes, $50 per Sickness deductible.
Q: With this insurance must I go to the University Health Center first?
A: No. However, students are strongly encouraged to use the resources of the
Student Health Service or infirmary first.
Q: What if I am out of state when I get sick or injured?
A: You will be insured for services wherever you are, 24 hours a day.
Q: How do I get an I.D. card?
A: An I.D. card is provided in the 2007–2008 Injury and Sickness Insurance Plan
brochure. Additional I.D. cards can be printed from this website as well.
Q: What is the policy’s maximum benefit?
A: Under the basic Accident portion of this plan, benefits are paid at a 100% of
Usual & Customary charges up to a $1,000 maximum benefit. The basic Sickness
portion of this plan provides coverage up to a $1,000 maximum, subject to a $50
per Sickness deductible, and paid according to the schedule of benefits that
appear in the 2007-2008 Student Injury and Sickness Insurance Plan brochure.
If the covered medical Expense for your Injury or Sickness exceeds the aggregate
maximum we owe under the basic Accident or basic Sickness benefits, we will pay
80% of the Expense up to the maximum benefit of $49,000. Covered Expenses for
daily Hospital room and board will not be more than the usual semi-private room
charge.
