Q: What is the policy number?

Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A: The plan is mandatory for all full-time residential students, international students, nursing program students and student-athletes, unless proof of other insurance coverage is submitted to the Student Assistance Center within the first 8 days of the semester. All other students enrolled in the School of Arts and Science may choose to enroll on an optional basis.

Q: Can I get the insurance to cover my spouse and/or children?
A: All students, except Accelerated Program Students who enroll in this coverage, may also enroll their eligible dependents in this coverage. Eligible dependents are the spouse residing with the Insured Student, and unmarried children under twenty-three (23) years of age who are not self-supporting and reside with the Insured Student.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or Sickness which occurs while you are covered under the program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure.

Q: Can I go to any doctor?
A: Yes you can. However, the Insurance company does work with First Health, a Preferred Provider Organization network of physicians and hospitals. Going to a First Health Network Physician or Hospital would in most cases result in lower charges which would directly lower any co-payments that you would have to pay. You can find a network doctor or hospital near you from this website.

Q: Where do I get a claim form?
A: You can print one from this web site (see "Claim Form"). You can also obtain one from Richard Liedtke in the Student Assistance Office at 816-584-6800 or one will be sent to you upon our first notice of a claim.

Q: How do I file a claim?
A: Follow the instructions on the front of the Claim Form. The Claim Form must be completed on both sides, including the “Statement of Other Insurance”. Bills must be received by Bollinger within 90 days of service.

Q: Does this policy cover me for my annual physicals?
A: No. However, certain preventive screenings are allowed, i.e. mammography, prostate cancer screening, etc. Please refer to brochure.

Q: Is there a deductible on this policy?
A: No.

Q: With this insurance must I go to the University Health Center first?
A: No, this plan does not require you to use the University Health Center first.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for non-excluded illnesses or Injuries which occur out of state. You must follow the same procedures in filing a claim as if it had happened here in Missouri.

Q: What is the policy’s maximum benefit?
A: $50,000 per Injury or Sickness

Q: What do you pay for doctor’s visits?
A: Non-surgical Physician’s Visits are paid as follows: $50 per visit, 1 visit per day.
